Ohio · Connections aftercare

Support through the first 30–90 days after treatment.

Leaving residential treatment can mean moving from a structured setting back into everyday life. Connections describes a bridge of outpatient supports and coordination for that transition.

The handoff window

A connected next step, month by month.

  1. Days 01–30

    Land softly

A warm handoff, a first check-in, and a clear plan for the first weeks home.

  2. Days 31–60

    Build rhythm

Therapy, medication support, groups, and coaching settle into a dependable cadence.

  3. Days 61–90

    Keep momentum

Signals and progress inform the next care step as the transition continues.

Why the handoff matters

The 30–90 day discharge gap

A discharge plan marks a change in setting, not the end of recovery support. The first weeks at home bring new routines and decisions, while follow-up care may involve several people and services.

Morrowkin describes Connections around the 30–90 days after residential treatment: a period for keeping therapy, medication support, groups, coaching, and coordination in view as people take their next steps.
